Marching Band Required Paperwork / Physical Information

In order to participate in UIL activities for the 2022-2023 school year, all Band Students must

1.) Parents and students must read, sign, and return the parent/student acknowledgement of rules form to the band staff.

2.) All 9th and 11th graders must complete the pre-participation and medical history form and return to the band staff. All 10th and 12th graders must complete the medical history form only and return it to the band staff.

Physicals can be obtained from any chiropractor or medical doctor. The Goodall Witcher Clinic will be offering free physicals to Bosque County students all summer but an appointment must be made for it to be free.

Please do not wait until the last minute to schedule your appointment for a physical or to complete the forms above. HS Marching Band begin practices on July 18th, so this needs to be completed as soon as possible.
